Release of Patient Records

Please contact SaraPath Diagnostics® Medical Records Coordinator at 941.362.8917 for assistance in sending patient records to another medical facility.  Our Medical Request For Consult Form is available here.
 

Slides and Blocks

Because laboratories are required to maintain and ensure the integrity of diagnostic materials, SaraPath is permitted to release or transfer slides or blocks only under the following conditions:
  1. To another CLIA accredited medical facility (for a consult or a second opinion)

  2. Under a duly executed subpoena or court order
All slides and blocks are sent by Federal Express (or other traceable carrier) directly to another medical facility. Materials can be sent same day or overnight if needed.
 

Pathology Test Results

You may obtain a copy of your pathology test results as follows:
  1. From Your Healthcare Provider:  Ask your healthcare provider to contact you when they receive your test results. Once you and/or your healthcare provider receive your results, you’ll want to discuss them together and decide on the appropriate treatment, if necessary.

  2. From SaraPath Diagnostics: You can obtain your test results directly from SaraPath. Please click “Get Test Results” in the menu to get started or give us a call to obtain a request form.

SaraPath Diagnostics is committed to transparency and we strive to provide our patients and third-party insurance carriers with timely and accurate billings. It is our goal to make sure that each patient understands his or her financial responsibility for pathology services and to provide assistance with financial aid options when needed. We encourage you to call our Billing Department with any questions or concerns regarding your bill for pathology services.
 
We receive billing information with your specimen from your doctor or referring facility. If you have insurance that was not billed, or you think that it was billed incorrectly, please contact us. We are contracted with most insurance plans. Additionally, you have rights and protections against surprise medical bills - When you get emergency care or get treated by an out-of-network provider at an in-network hospital or ambulatory surgical center, you are protected from surprise billing or balance billing.
